CalPERS owns and manages over 1,200 forms. Each time a member or employer needs to elect or change an option, such as designating a beneficiary or applying for service retirement, a transaction with CalPERS, by way of a form, is required.

In 2019, a senior leader I partner closely with, led the initiative to formalize a policy for our external forms. With the resurrection of the Forms Management Unit, the policy defined the roles, responsibilities, and objectives for the maintenance and alignment of our forms.

Through subsequent phases of the External Forms project, I have been closely involved with the design and implementation of a MS Word form template that meets the requirements for accessibility (WCAG 2.1) and the California Department of General Services’ State forms standards.

Following both the State of California forms standards and our CalPERS brand and graphic guidelines, I steered the design type, hierarchy, and various formatting styles with the Digital Accessibility Coordinator.

Understanding the digital accessibility law and requirements is fundamental to this design. The goal is to empower forms owners to create, maintain, and publish accessible forms that are consistent, accurate, and functional.

To accompany the form template, I wrote and designed a visual guide. These are sample pages, which feature resources, form composition, and formatting styles.
